INFORMED CONSENT
The purpose of these sound-based and mindfulness-informed sessions is to support your emotional, mental, physical, and spiritual well-being through a blend of therapeutic sound practices, meditation and principles of music therapy. These sessions aim to help you explore and address personal challenges, reduce stress and anxiety, develop inner awareness, and enhance your overall quality of life. As a music therapy student and sound healing practitioner, I use a holistic approach that may include sound meditation, breathwork, guided imagery, intentional listening, and live or recorded sound to support your self-exploration and healing.
Nature of Sessions
Voluntary Participation: Your participation in these sessions is entirely voluntary. You may pause or discontinue at any time.
Confidentiality: All personal information shared in sessions will be kept confidential unless disclosure is required by law (e.g., risk of harm to yourself or others).
Risks and Benefits: Engaging in sound-based therapy may bring up unexpected emotions, memories, or sensations. While this can feel intense at times, the process is generally experienced as grounding, calming, and clarifying. Many clients report improved emotional regulation, stress reduction, increased focus, self awareness, and spiritual insight.
Practitioner's Responsibilities
As your practitioner and a music therapy student, I commit to:
Providing a safe, supportive, and non-judgmental environment
Respecting and protecting your confidentiality
Using evidence-informed and ethically grounded approaches within the scope of my training
Seeking supervision and guidance from qualified faculty or professionals as part of my ongoing development
Referring you to appropriate professionals if your needs fall outside my scope of practice
Client’s Responsibilities
As a client, you are encouraged to:
Communicate openly about your experiences, comfort levels, and goals
Let me know if anything in the session feels overwhelming or uncomfortable
Participate actively and reflect on the practices or insights that emerge
Inform me of any mental health or medical conditions that may affect your experience
Confidentiality and Limits
I will maintain your privacy and confidentiality in accordance with ethical standards and university policy. However, I am legally and ethically obligated to break confidentiality under the following circumstances:
If there is a clear risk of harm to yourself or others
If there is suspected abuse or neglect of a child, elder, or vulnerable adult
If I am required to comply with a court order or legal process
In these instances, I will make every effort to inform you before taking any action.
Duration and Frequency of Sessions
Sessions are typically 60 minutes in length and may occur weekly, bi-weekly, or as agreed upon. The total number of sessions will be collaboratively determined based on your needs, goals, and availability. These sessions are not intended to be a substitute for long-term psychotherapy or psychiatric care.
Fees and Payment
Session Fee: $90
Payment Method: Cash / Card
A minimum commitment of 4 sessions is required to begin this process. As part of this package, clients will receive a 5th session at no additional cost.
Clients may choose from the following payment options:
Pay in Full: $300 due following the initial session.
Installment Plan: Payment for the first two (2) sessions is due after the initial session. Payment for the remaining two (2) sessions is due following the completion of the third session.
By selecting either payment option, the client agrees to complete the full series of four sessions. This structure ensures continuity and supports the effectiveness of the healing process.
Cancellation Policy
I understand that there is a 24-hour cancellation policy
I understand that if I LATE CANCEL, I will be charged a $35 LATE CANCEL FEE
I understand that if I NO CALL/NO SHOW I will be charged a $45 NO SHOW FEE
Client Rights
As a client, you have the right to:
Ask questions about any aspect of the session or practitioner’s background
Decline any activity or intervention without judgment
Request referrals for additional or alternative forms of support
Withdraw consent and discontinue services at any time
